Semaglutide carries a boxed warning related to thyroid C-cell tumors observed in rodents. It should not be used in patients with a personal or family history of medullary thyroid carcinoma (MTC) or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Patients should discuss neck masses, persistent hoarseness, trouble swallowing, or other concerning symptoms with their healthcare provider.
Tell your healthcare provider if you are pregnant, planning pregnancy, breastfeeding, have a history of pancreatitis or gallbladder disease, have significant gastrointestinal problems, or take medications for diabetes. Semaglutide should be discontinued at least 2 months before a planned pregnancy because of its long washout period.
Because semaglutide delays gastric emptying, tell healthcare professionals that you use semaglutide before surgery or procedures requiring anesthesia or deep sedation.
Compounded semaglutide products can differ in concentration and packaging. Follow the pharmacy label and prescriber’s directions exactly. Do not convert milligrams, milliliters, or syringe “units” without specific instructions from the prescriber or dispensing pharmacy.
Weeks 1–4
Treatment commonly begins with a lower prescribed dose to improve gastrointestinal tolerability. Appetite may begin to decrease, while nausea, fullness, constipation, diarrhea, or other gastrointestinal effects may occur as the body adjusts.
Weeks 5–8
If tolerated and prescribed, the dose may be increased gradually. Patients may notice more consistent appetite control and earlier fullness. Weight response varies and should be evaluated along with nutrition, activity, tolerability, and overall health.
Weeks 9–16 and Beyond
Ongoing treatment may support continued weight-management progress when combined with a reduced-calorie eating pattern, physical activity, and medical follow-up. The appropriate dose and duration are individualized by the prescriber.
Use the medication exactly as directed and keep scheduled follow-up appointments so the healthcare team can assess response, side effects, and whether dose adjustments are appropriate.
If a dose is missed, follow the instructions provided by the prescriber and dispensing pharmacy. Do not take extra medication or change the dose to make up for a missed dose unless specifically directed.
